What is LiveWell Senior Care Advantage?

East Texans with a Collom & Carney Clinic or Diagnostic Clinic of Longview primary care physician and enrolled in a Humana Medicare Advantage plan, Aetna Medicare Advantage plan or BlueCross BlueShield of Texas Medicare Advantage plan will have an opportunity to participate in LiveWell Senior Care Advantage.

LiveWell Senior Care Advantage is a program built on the premise that through stronger connections between patients and primary care physicians; between health plans and those caring for patients; and between doctors who treat the whole person and those who specialize in a specific procedure or condition, care can be transformed.

LiveWell Senior Care Advantage is not a new Medicare Advantage plan, rather, it is a new approach to care delivery which focuses on preventative care, care coordination resources, when you need them, and an appreciation for the demands and constraints on your desired lifestyle. The primary care physicians at Collom & Carney Clinic and Diagnostic Clinic of Longview are committed to supporting you in reaching your health and wellness goals.

LiveWell Senior Care Advantage patients are encouraged to complete an annual wellness visit with their primary care physician. This comprehensive, individualized assessment provides an annual roadmap to improved health and wellness. It is also the gateway for our patients to access specialized programs designed for those who need more focused care coordination resources.

A licensed Medicare broker is a professional independent of any specific health plan and is appointed to represent multiple health plans. They can review your specific needs and help you compare Medicare coverage options for this enrollment year.

We can put you in touch with brokers who understand the LiveWell Senior Care Advantage program. These brokers can help you evaluate coverage options to find one that meets your needs. With your permission, we will give your contact information to a broker to contact you to discuss coverage options.
